Health2040 addresses the anticipated transformations in outpatient primary healthcare in Switzerland, driven by demographic shifts, rising rates of chronic illness, cost pressures, increasing demand for health services, and technological advancements.
Co-Creation Process and Action Planning
In 2023, Health2040 initiated a co-creation process with over 50 stakeholders from healthcare, social services, and civil society to shape a vision for sustainable outpatient primary care. Through a structured co-creation methodology, participants collaboratively developed:
A care model that reflects patient-centered principles and anticipates societal and healthcare needs.
An action plan that provides strategic directions for implementing this care model effectively.
The co-created action plan emphasizes a comprehensive, adaptable care network, called the “Gesundheitsnetz für alle – pour tous – per tutti” (Health Network for All). The model aims to integrate healthcare, social services, and civil society, leveraging digital tools and ensuring access across diverse population groups.

Health2040 is a collaborative project between the University of Lucerne’s Faculty of Health Sciences and Medicine and Interface Lucerne, under the umbrella of the Swiss Learning Health System (SLHS).
The Health2040 framework includes several key projects:
Population Preferences Survey : Capturing the preferences of the general Swiss population concerning future primary healthcare models.
Health Professionals Preferences Survey: Exploring the preferences of healthcare professionals regarding future models in primary care.
Task Shifting Study: An in-depth investigation into the facilitators and barriers of task shifting among healthcare professionals, examining how specific roles and responsibilities can be shared to improve service delivery.
